Tween Session Questionnaire

We want to thank you for scheduling your tween's session with us.  We are excited for the opportunity to show you and your tween just how amazing they are at this time in his/her life.

To help us better prepare for your portrait session, we have two questionnaire forms that we would like you to fill out prior to your appointment.

One is for our parents and grandparents and one is for the tween.  We ask that both parents fill out the form and if grandparents wish to as well, we strongly encourage that.
